Accessible Text Agreement

By submitting this agreement, I agree to the following:

• To use audio and digital copyrighted materials for my educational use only
• Not to otherwise share, reproduce, sell, transmit, publish, broadcast, disseminate or distribute the accessible digital files (or any version modified for accessibility purposes) to anyone including but not limited to other students with a disability
• Provide proof of purchase (e.g a purchase receipt) of the text requested
• Provide proof that the materials will be used only in the course in which you are registered
• Notify Disability Support Services of any changes in class schedule or course materials which affect book request immediate by e-mail (dss@bowiestate.edu)

Please Note

• Exchange of any accessible digital files- alternate text, media and audio produced by the Disability Support Services or received from the office or in a modified format or copy is a violation of the U.S Copyright Laws
• You will be notified via email when your materials are ready
• Requests will be processed in order in which it was received
